Member Login

There are three ways to submit a claim

  1. Mobile App: Search the Apple App Store or Google Play for Surency Flex.
  2. Member Account: Click here to log in to your account. Then click “File A New Claim”.
  3. Paper Claim FormClick here to download the Health Care FSA claim form. Click here to download the Dependent Care FSA Form. Fill out the appropriate form and submit it using the information on the form.

Surency Flex Mobile App

Want to check your account balances and submit receipts anywhere, anytime? We have an app for that!

Access the information you need:

  • Check Health Care Flexible Spending Account (FSA), Dependent Care Flexible Spending Account (DC FSA), Health Reimbursement Arrangement (HRA), Lifestyle Spending Account (LSA), Health Savings Account (HSA), and Commuter Benefits account balances
  • View account activity
  • Access FSA Store to buy eligible expenses
  • Use the Eligible Expense Scanner to see if what you're about to purchase is an eligible expense

Take action:

  • Submit claims for Health Care FSAs, Dependent Care FSAs, HRAs, LSAs and Commuter Benefits accounts
  • Snap a photo of receipts within the app to submit with new or existing claims
  • Add banking information 
  • Request HSA distributions and make HSA contributions
  • Access account funds to pay yourself back or to pay your doctor
  • Report a Surency Flex Benefits Card as lost or stolen

Troubleshooting

  • If you exceed the maximum number of failed logins, your account will be locked for security purposes. Contact Surency at 866-818-8805 for assistance or click the “Forgot Password” link within the Member Account.
  • If you recieve a message about setting up additional security, log in to the Member Account and answer the five security questions.
