LASER EYE SURGERY DISCOUNTS
Laser eye surgery discounts are available for all laser vision correction treatments including PRK, LASIK, Custom LASIK, and bladefree. The U.S. Laser Network includes access to more than 440 LASIK surgeons at over 600 locations across the United States*.

This discount applies to the global cost of the treatment, surgeon fee, facility fee, and all pre- and post-operative care. All laser vision correction procedures are included (no exclusions) and after discount, prices will not exceed**:

ACCESS YOUR DISCOUNT
- Locate the U.S. Laser Network provider of choice by calling 877-5LASER5 and identify yourself as an EyeMed member (Surency Vision’s network partner). You will receive a discount certificate and instructions. Call center hours are Monday-Friday 8 am to 8 pm EST and Saturday 9 am to 5 pm EST.
- Schedule a consultation with the doctor of your choice. If you’re a good match for surgery and decide to proceed, a treatment date will be scheduled.
- To activate the discount, call 877-5LASER6 and a representative will issue you an authorization number confirming the discount. At this time, you will remit a deposit, which will be applied to the total cost of the procedure. A patient voucher authorizing the discount will be sent to the provider prior to treatment.
- On the day of the procedure, you should pay or arrange to pay the balance of the fee.
LASIKPLUS VISION CENTERS
There are LasikPlus Centers nationwide, and they represent a select sub-set of the U.S. Laser Network. They offer additional benefits to members, including:
